Paperwork

The examination will be carried out in accordance with the requirements of the relevant licensing authority's medical form. For HGV, LGV and PCV licence holders this will be completion of the DVLA D4 Medical form.

Taxi drivers will usually be issued with a specific medical form devised by their local Council, although some councils that we carry Taxi medicals out for require on a DVLA D4 from being completed.

Motorsport medicals can vary depending on relevant race Licence body. These again will be provided with a medical form for the doctor to complete. Motorsport medicals may require an ECG so please talk to us directly before booking this type of medical.


What your Medical entails

  • Take a Blood pressure measurement
  • Perform an eye test with and without glasses (if required/worn)
  • List on the D4 Form any medication you're currently taking
  • Ask about questions about your medical history. The doctor will need to understand and be able to discuss with you your complete medical history

The completed D4 Medical form must be posted to the DVLA. The ultimate decision to grant or renew the Licence is their decision.

Should the Doctor identify any serious medical condition or issue this will be discussed with you at the time of the examination and offered advice how it would affect your application. You may proceed with the medical but the Doctor will not sign off the medical form.


HGV / LGV Medical

The medical requirements for an HGV/LGV Licence is identical to that for PCV: DVLA D4 form must be completed and a medical examination must be passed when first applying for your HGV Licence at 18; then a further medical examination at age 45 and thereafter at 5 yearly intervals until reaching the age of 65 when annual medicals are required.

The interval between having your medical examination and applying for your Licence (or Licence renewal) must not exceed 3 months. The DVLA must be made aware of any changes to the health of an HGV / LGV Licence holder at any time. This may result in a requirement for a further medical examination being carried out depending upon individual circumstances.

PCV Medical

A restricted bus Licence can be obtained at age 18 (route and distance restrictions apply). A full bus Licence can be obtained at age 21.

By law a DVLA D4 form must be completed for both types of PCV Licence and a relevant medical examination must be passed before a Licence can be granted.

Taxi Medical

If you wish to apply for a taxi Licence a medical examination should take place before the application stage and then when the renewal of your Licence is due.

The DVLA expects that a taxi driver should meet the same medical requirements as a HGV or PCV Licence holder. Visit the DVLA website if you need any more information at www.dvla.co.uk or speak with your local council.

Please ensure that you bring the relevant Licensing Authority Medical Assessment Form with you otherwise the medical cannot be completed.
